This proposed project develops, implements, and evaluates a collaborative “Data-to-
Care Rx” model using pharmaceutical insurance claims data to identify persons with
HIV in real-time who fail to pick up prescribed ARV medications by 30 days and 60
days, and to implement targeted evidence-based interventions at each of these
benchmarks in an effort to address adherence barriers and re-engage these individuals
in HIV medication therapy and primary care. This Data-to-Care collaboration is made up
of four partnering organizations that will collaborate with the Centers for Disease Control
and Prevention (CDC) – Amida Care (AC), Cicatelli Associates, Inc. (CAI), the
Brookdale Center for Healthy Aging at Hunter College of the City University of New York
(CUNY), and the New York State Department of Health (NYSDOH) AIDS Institute. AC
has real-time access to pharmacy claims data, medical visit claims data, HIV viral load
and CD4 measure laboratory data, and has developed ongoing relationships with the
provider community. This access, in combination with the expertise of the CDC, the
evaluation expertise of CUNY, and the expertise and support of the NYSDOH, will
enable the identification of persons who fail to fill ARV prescriptions in real-time and will
facilitate timely interventions to occur prior to a gap in care or loss to care that could
have a significant impact on ARV adherence, HIV suppression, and retention in care.
